We are writing to inform you that we have made the difficult decision to close West Linn Family Health Center, effective March 19, 2026. Please carefully read the enclosed information regarding transitioning your medical care and accessing your medical records.

Dr. Endo, Dr. Scott, and Dr. Robinson plan to continue practicing medicine in the area but those specifics are not known at this time. Providing care for you has been such a privilege for the three of us and we value the trust you have placed in us over the years. Thank you again for allowing us to serve as your physicians. KEY DETAILS TO TRANSITION YOUR MEDICAL CARE
As West Linn Family Health Center (“WLFHC”) is closing, it is important that you begin the process of selecting a new primary healthcare provider immediately. Our last day for seeing patients and providing medical care will be March 19th, 2026. We will do our best to accommodate you as possible as you transition your care outside of our clinic. Any labs or test orders that have not been completed by March 19th, 2026 will need to be ordered by your new primary care provider. After our closing date, you should contact your new physician or an urgent care facility for any health care needs. If you feel you have a serious, life-threatening illness or injury that requires immediate medical care, call 9-1-1 or go to the nearest emergency department or hospital.

TRANSFERRING AND ACCESSING YOUR MEDICAL RECORDS
Your health and the continuity of your medical care remain our top priority. Your medical records are confidential, and a copy can be transferred to another doctor or released to you or another person you designate only through your written authorization. Please use the enclosed authorization form so we may transfer your records to your new physician. Throughout this process, your medical records will be securely stored. Through our closure date, you may request a copy of your medical records from WLFHC directly. After our closure date, information on how to obtain a copy of your records will be available online at www.wlfhc.com and (503)-635-8384.

RECOMMENDATION FOR FINDING A NEW PHYSICIAN
We strongly encourage you to establish with a new primary care physician as quickly as possible to avoid any gap in your medical care. If you need any assistance finding a new primary care physician, we recommend that you contact your insurance for a list of in-network providers accepting new patients.
